Dear Collaborative Members & Visitors:
One of the kindest gifts that you can give to your clients and patients is to help facilitate their completion of an Advance Health Care Directive which designates an individual to speak for them if they are unable to communicate in critical medical decisions. Advance Directives:
- Eliminate the shock and ambiguity thrust upon family in medical emergencies when they are often required to make critical decisions for a loved one without a clear statement of that person’s desires
- Allow for decisions ranging from “Prolong my life as long as possible . . .” to “Don’t prolong my life if . . .”
- Are easily cancelled and revised and are simply a clear statement to family and medical community of what the individual wants to have happen in case they can’t speak for themselves.
